Required Elements of a Valid Notice

• Identification of the copyrighted work claimed to be infringed.
• Identification of the allegedly infringing material with the exact URL or URLs.
• Contact information for the complaining party.
• A good-faith statement that the disputed use is not authorized by the owner, the owner's agent, or the law.
• An accuracy statement made under penalty of perjury.
• A physical or electronic signature of the rights holder or authorized agent.
